Typically, when you are injured on the job, you know exactly what happened. It was an industrial accident, you cut yourself, you were in a car accident, you got a broken bone, what have you – you know exactly what happened and when the injury occurred. Within 90 days from that point, you have to file a request for compensation form – a C-4 form.
