## How to File a Workers’ Compensation Claim in Nevada

Workers’ compensation provides financial support for workers injured on the job. In Nevada, any employer with one or more employees must carry workers’ compensation insurance. 

If you have suffered an injury or illness while working in Nevada, it is important to understand your rights and how to make a successful workers’ compensation claim.

- **Report the workplace injury** to your employer immediately. You must notify your employer within seven days of the injury in all cases, but doing so immediately is beneficial.
- Your employer must notify the insurance company of the accident and file an accident report with the [Nevada Division of Industrial Relations](https://dir.nv.gov/).
- **Do not wait to get care**. In an emergency, go to the emergency room immediately or call an ambulance. In all other cases, ask your employer where to go to get care and do so immediately. Continue all follow-up care provided by your doctor.
- Your employer will complete an Employer’s Report of Injury (C-3) and submit it to the insurer. Your treating medical provider will complete the C-4 Claim for Compensation form, which officially initiates your workers’ compensation claim. The workers’ compensation claim must generally be filed with the insurer within 90 days of the date of your injury.
- After the claim is filed, the insurer will review it and determine whether to accept or deny your workers’ compensation benefits.

### The C-4 Form

The [C-4 form](https://dir.nv.gov/uploadedFiles/dirnvgov/content/WCS/c-4.pdf) is the treating medical provider’s initial report of your injury and claim for compensation. To support your case, make sure your provider has accurate and complete information about the accident, your job duties, any witnesses, and the medical treatment you received.

You must report your injury to your employer within seven days of the accident. Your treating medical provider will complete and submit the C-4 form to initiate your claim. Your employer’s insurer will receive the C-4 and process your claim.
